Please download the form and fill it with appropriate data and print it.

Note:
1) For Mardo/Bairo : Zakat ul Fitr and Silat-ul-Iman MUST be filled
2) For Gair Balig and Amvaat, Hamal, Amvaat : ONLY Zakat-ul-Fitr has to be filled
3) Add up all the Zakat-ul-Fitr and Silatul-ul-iman and put under "Total A+B"
4) Please try to "Round off" the amount for ease of accounts
